Punctuation:
Punctuation is not used except for abbreviations such as "Mr. or Mrs." and for separating a phrase
Numbers:
Numbers are written out except for the street address; the number is written out if the number is less than 10
The Request:
Ceremony is located at a house of worship
request the honour or your presence
Ceremony is located outside a house of worship
request the pleasure of you company
The Names:
The bride and groom's middle names are included, however, their parents' middle names are not necessary
Mister (Mr.) and Misses (Mrs.) are abbreviated, however, Doctor, Reverend, and Captain should be written in full
The Date:
The year should not be capitalized' it is not a proper noun (most common mistake)

The Time:
On the hour ("evening" begins at six o'clock)
at five o'clock in the afternoon
at six o'clock in the evening
On the half hour (remove "in the ____" from the end of the line)
half after four o'clock"
or
at half past two o'clock
The Address:
Directionals are spelled out before the street name, abbreviated if after the street name
123 West Main Street
or
123 Main Street, N.W.
City, State, Zip Code (states are spelled out; zip codes are only used on the envelopes)
